Below is an agreement form that I send to all my clients to review and sign. In order to obtain the desired results one would hope to achieve in a coach/client relationship, a strong level of commitment from both parties is vital.


I, the Coach, agree to the following: 


I aim to provide a high-quality professional coaching service, which meets the needs and interest of my clients. I aim to build a meaningful relationship with my clients, which in turn, will enable us to work together to enhance your decision-making skills and competencies. I aim to support you in achieving your goals, career, health, wealth, contribution, relationships, and mission in this world.


Respect, Integrity, Honesty, Quality, Professionalism, Partnership, and Value.

It is not required that my clients be of the same faith as myself, but it is understood that I will apply the Gospel and Biblical truth in my coaching practice. I will not, however, impose these beliefs onto my clients. It is my strong stance that an essential element to professional coaching is the coaches ability to guide their clients toward self-discovery. Life coaching is not about giving advice, but guiding clients toward answers they discover themselves.


I will make every effort to supply the coaching hours contracted to high standard and with utmost professionalism. Should I fail, I will seek to remedy the mistake to your satisfaction.


I will at all times respect the confidentiality of my clients, and will not divulge personal information unless required to by law.

I, the Client, agree to the following: 


I understand that the level of my commitment is in direct correlation to my results. If at any time my coach feels that my commitment level does not match the outcomes/goals I desire, it is the coaches right to refer me elsewhere.

I understand that my coach’s worldview comes from a Gospel centered belief system, and she will incorporate Biblical concepts into our coaching session as a means of providing supported understanding and awareness to our topic of conversation, but not as a form of coercion. Just as my coach will respect my personal worldview, so I will respect my coach’s.

I will show up, and fully engage during our coaching sessions. I will also fulfill the homework assignments between sessions, to the best of my ability. I understand that failure to show up for an assigned session will still require payment in full (unforeseen emergencies will be considered on a case by case basis). If I need to reschedule a session I will do so a minimum of 48 hours prior to the scheduled date. Failure to notify my coach within this time frame will result in a full payment of the missed session.

Coaching sessions are conducted by phone, Skype, or in person if you live in the greater Seattle area. A three-month commitment is strongly recommended up front, although we can discuss month-to-month coaching. Experience has shown me that the higher the level of commitment, the greater the results. The total investment for your personalized coaching program will be discussed during the initial consultation call, which is free of charge. Your individual coaching package includes:

  • Two sessions per month/meeting every other week (6 sessions total). Please note: coaching sessions take place during a scheduled period of time (usually 60 minutes in length), but the unscheduled hours I commit to each client by way of note taking, as well as professional development, adds additional hours each client’s overall care.
    • Email correspondence in between sessions, when necessary
    • Initial intake questionnaire that you complete and we review before our first session
    • A copy of my book, The Chance to Choose, which introduces The Jeremiah Method ™ and provides homework and reading assignments to keep you focused and growing
    • Confidentiality and safety to share whatever is on your mind

Payment is required up front (I can work with you on a payment plan) and the cancellation policy is 48 hours in advance. Payment is accepted via personal check, or cash if meeting in person.

Fee structure:

An initial 45 minute phone consultation is free of charge, after email correspondence and an intake evaluation form is completed by the potential client, and reviewed by the coach.

Platinum (best value): 6 calls-1 hour duration ($450/$75 per hr)

Gold: 6 calls-45 minute duration ($360/$60 per ¾ hr)

Silver: 6 calls-30 minute duration ($270/$45 per ½ hr)

I only work with a limited number of clients at a time, and who are committed to being held accountable, showing up 100% for each session, and willing to be break out of their comfort zone.
